Community providers focus on prevention, patient care coordination, and improved quality of care, thus improving patient experience and health outcomes.
Who We Are
AZCares is an Accountable Care Organization that contracts with health plans, medical institutions, governmental agencies and private organizations for value-based services. The purpose of value-based contracting is to provide access to quality healthcare and improve the health of patients and patient support.
AZCares manages value-based services by supporting a network of community providers who collaborate and work together to achieve collective healthcare goals. In partnership with Arcadia Solutions, AZCares provides real-time data analytics to inform clinical practices and drive positive patient outcomes.
AZCares incorporates data analytics into daily practice to better assess and enhance services and the patient’s experience. All staff delivering services under the AZCares network are highly trained and committed to quality care.
